2. With excessive use of opioids and some other substances

I can see how peoples' natural pain relieving chemicals could be depleted.

As for the theory that depression, anxiety and other mental conditions don't lead to abuse of 'feel good' substances and addiction, but instead unhealthy dependence is caused by overabundance, I don't know about that. A few people I've seen fit the first description to a T, even without any family members in the same situation- relatives who had easy access to the same 'overabundance of substances.'

I'm also thinking what's the reason people with severe injures, those about to undergo surgery and others in physical distress often need sedation and 'pain medication'?

I'll read the book maybe and see what the author says in interviews.
