The death of a 53-year-old worker at a bowling alley has prompted a U.S. Department of Labor investigation.

In November 2014, David Geiger, who worked at Northwest Lanes in Fairfield, died after his sweatshirt got caught in a pinsetter and he was asphyxiated, according to news reports.

The U.S. Department of Labor's Occupational Safety and Health Administration investigated the death and found that the machine's operating parts were improperly exposed. The findings led to the administration to propose penalties of $45,500.

"A wife, children and a grandchild lost a loved one in a preventable workplace tragedy," said Bill Wilkerson, OSHA's area director in Cincinnati.
